Accident Investigation
Accidents happen, that’s a fact!
Is your bank prepared to gather and retain information needed for a report?
Here a few investigation tips:
- Use an unbiased approach during the investigation.
- Interview witnesses and injured employees at the scene.
- If possible, conduct a walkthrough of the accident.
- Conduct interviews in private, one witness at a time.
- If possible, keep the witnesses separate until after your interview.
- Obtain signed statements from all involved.
- Take photos and make sketches of the accident scene.
- Determine what hazards were present and what unsafe areas contributed to the loss.
- Ensure hazardous conditions are corrected.
- Keep a copy of all field notes.
