Accidents happen, that’s a fact! 

Is your bank prepared to gather and retain information needed for a report?

 

Here a few investigation tips:

  • Use an unbiased approach during the investigation.
  • Interview witnesses and injured employees at the scene.
  • If possible, conduct a walkthrough of the accident.
  • Conduct interviews in private, one witness at a time.
  • If possible, keep the witnesses separate until after your interview.
  • Obtain signed statements from all involved.
  • Take photos and make sketches of the accident scene.
  • Determine what hazards were present and what unsafe areas contributed to the loss.
  • Ensure hazardous conditions are corrected.
  • Keep a copy of all field notes.
